RESUMO Objetivo Descrever o efeito da conduta fonoaudiológica no processo de decanulação traqueal em indivíduos com traumatismo cranioencefálico (TCE). Método Estudo clínico transversal prospectivo controlado. Participaram deste estudo dois grupos de indivíduos com TCE confirmado por tomografia axial computadorizada, sendo o grupo de estudo (G1) e o grupo controle (G2) compostos por 30 indivíduos cada, com 25 (83,3%) indivíduos do gênero masculino e 5 (16,7%) do gênero feminino em cada grupo. A faixa etária variou de 18 a 53 anos, com média de 32 anos. Foi elaborado um instrumento de avaliação fonoaudiológica para indivíduos com TCE traqueostomizados composto por investigação do nível de consciência, cognição e deglutição (anexo 1) e conduta. O G1 recebeu a avaliação proposta pelo estudo e o G2, análise retrospectiva de prontuário sem avaliação fonoaudiológica. As variáveis, tempo de permanência com a traqueostomia e total de dias de internamento foram os marcadores de mensuração do efeito da conduta fonoaudiológica com esse instrumento nessa população. Resultados Verificou-se que o G1 obteve uma redução média de 4,2 dias de permanência com a traqueostomia e de 4,4 dias no tempo de internamento hospitalar quando comparado com G2, porém sem significância estatística (p = 0,2031). Conclusão O grupo que foi avaliado e recebeu a conduta fonoaudiológica proposta no instrumento obteve diminuição do tempo de permanência com a traqueostomia bem como redução do tempo de internamento hospitalar.

ABSTRACT Purpose To describe the effect of Speech-Language Pathology (SLP) management on the tracheal decannulation process in patients with traumatic brain injury (TBI). Methods Prospective controlled clinical study. Two groups of patients with TBI confirmed by computed axial tomography were included in the study group (G1) and control group (G2) composed of 30 individuals each, with 25 (83.3%) male and 5 (16.7%) female individuals in both groups. Patients’ age ranged from 18 to 53 years old – mean age was 32 years. A SPL assessment tool was developed for tracheostomized patients with TBI, composed of investigation of awareness level, cognition and swallowing (annex 1) and conduct. G1 underwent the assessment proposed by the study, and G2 was assessed by retrospective analysis of medical records without SLP evaluation. In this population, the variables time with tracheostomy and total days of hospitalization were the measurement markers for the effect of SLP conduct with this instrument. Results It was verified that G1 presented mean reduction of 4.2 days with tracheostomy and of 4.4 days in length of hospital stay when compared to G2. However, these figures are not statistically significant (p = 0.2031). Conclusion The group that was evaluated and received the SLP conduct proposed in the instrument presented a reduction in the time of permanence with tracheostomy, as well as in hospital stay.

Abstract Introduction Dysphagia is a common symptom in children with cerebral palsy, either in oral or pharyngeal phases. Children who face such difficulties tend to show health problems such as food aspiration, malnutrition and respiratory infections. Videofluoroscopic swallowing study is the most recommended for these cases, as it reveals the real situation during swallowing. Objective The study aimed to verify the occurrence of aspiration pneumonia in children with cerebral palsy after videofluoroscopy. Methods The population for this prospective cross-sectional study involved 103 children with cerebral palsy, referred for videofluoroscopic who had returned for medical examination after a week to search for signs and symptoms of pneumonia. Results The study involved 46 girls (44.66%) and 57 boys (55.34%), aged between 0 and 14 years of age. Of the total, 84 (81.5%) had dysphagia, of which 24 (23.3%) were severe, 8 (7.7%) were moderate and 52 (50.4%) were mild dysphagia. None of the children presented aspiration pneumonia or infectious complications during the course of videofluoroscopy or after the procedure. Conclusion In the population studied, the authors found no cases of aspiration pneumonia, even with tracheal aspiration present in 32 (31.07%) cases.

Introduction The frequency of tracheostomy in patients with traumatic brain injury (TBI) contrasts with the lack of objective criteria for its management. The study arose from the need for a protocol in the decision to remove the tracheal tube. Objective To evaluate the applicability of a protocol for tracheal decannulation. Methods A prospective study with 20 patients, ranging between 21 and 85 years of age (average 33.55), 4 of whom were women (20%) and 16 were men (80%). All patients had been diagnosed by a neurologist as having TBI, and the anatomical region of the lesion was known. Patients were evaluated following criteria for tracheal decannulation through a clinical evaluation protocol developed by the authors. Results Decannulation was performed in 12 (60%) patients. Fourteen (70%) had a score greater than 8 on the Glasgow Coma Scale and only 2 (14%) of these were not able to undergo decannulation. Twelve (60%) patients maintained the breathing pattern with occlusion of the tube and were successfully decannulated. Of the 20 patients evaluated, 11 (55%) showed no signs suggestive of tracheal aspiration, and of these, 9 (82%) began training on occlusion of the cannula. The protocol was relevant to establish the beginning of the decannulation process. The clinical assessment should focus on the patient's condition to achieve early tracheal decannulation. Conclusion This study allowed, with the protocol, to establish six criteria for tracheal decannulation: level of consciousness, respiration, tracheal secretion, phonation, swallowing, and coughing.

Introduction  Stroke is a major cause of death and disability worldwide, with a strong economic and social impact. Approximately 40% of patients show motor, language, and swallowing disorders after stroke. Objective  To evaluate the use of software to infer the probability of pneumonia in patients with ischemic stroke. Methods  Prospective and cross-sectional study conducted in a university hospital from March 2010 to August 2012. After confirmation of ischemic stroke by computed axial tomography, a clinical and flexible endoscopic evaluation of swallowing was performed within 72 hours of onset of symptoms. All patients received speech therapy poststroke, and the data were subsequently analyzed by the software. The patients were given medical treatment and speech therapy for 3 months. Results  The study examined 52 patients with a mean age of 62.05 ± 13.88 years, with 23 (44.2%) women. Of the 52 patients, only 3 (5.7%) had a probability of pneumonia between 80 and 100% as identified by the software. Of all patients, 32 (61.7%) had pneumonia probability between 0 and 19%, 5 (9.5%) between 20 and 49%, 3 (5.8%) between 50 and 79%, and 12 (23.0%) between 80 and 100%. Conclusion  The computer program indicates the probability of patient having aspiration pneumonia after ischemic stroke.

INTRODUÇÃO: Estima-se 30.000 a 40.000 novos casos de paralisia cerebral, por ano, no Brasil. Os transtornos motores causados pela paralisia cerebral podem acarretar alterações na deglutição uma vez que alteram as fases preparatória, oral, faríngea e esofágica. OBJETIVO: Identificar os métodos de reabilitação existentes, na área da disfagia, nos casos de paralisia cerebral, com ênfase na busca por pesquisas que utilizaram os métodos neuroevolutivo Bobath, método Rodolfo Castillo Morales, terapia sensório motora orofacial e educação continuada. SÍNTESE DOS DADOS: Foi realizada uma revisão sistemática da literatura médica e fonoaudiológica sobre a reabilitação da disfagia orofaríngea em crianças com paralisia cerebral, abrangendo o período de 1977 a 2010, sem exclusão por língua ou nacionalidade. Dentre os 310 artigos encontrados, apenas 22 (7,09%) abordavam a atuação fonoaudiológica nas disfagias orofaríngeas em crianças com paralisia cerebral. Das 22 pesquisas encontradas 12 (54,5%) são do Canadá, 3 (13,6%) do Japão, 2 (9%) do Brasil, 2 (9%) da Alemanha, 1 (4,5%) dos EUA, 1 (4,5%) do Reino Unido e 1 (4,5%) da Polônia. 63,6% utilizaram a terapia sensório motora orofacial como método terapêutico, 36,3% mencionaram a educação continuada como forma de abordagem terapêutica, e apenas 18,1% e 9% utilizaram o método Bobath e o método Rodolfo Castillo Morales, respectivamente. CONCLUSÃO: Mesmo com uma população de portadores de paralisia cerebral aumentando constantemente ainda são poucas as pesquisas que englobam a (re)habilitação destas crianças no que diz respeito aos tratamentos das disfagias orofaríngeas.

INTRODUCTION: There are an estimated 30,000-40,000 new cases of cerebral palsy per year in Brazil. Motor disorders caused by cerebral palsy can lead to dysphagia as they may alter the preparatory, oral, pharyngeal, and esophageal phases. AIM: To identify existing rehabilitation methods of swallowing disorders in cerebral palsy, with emphasis on the pursuit of research using the Bobath concept, the Castillo Morales concept, oral sensorimotor therapy, and continuing education. SUMMARY OF THE FINDINGS : We performed a systematic review of the medical and speech therapy literature on the rehabilitation of oropharyngeal dysphagia in children with cerebral palsy spanning 1977-2010 and from all languages and nations. Among the 310 articles retrieved, only 22 (7.09%) addressed therapeutic rehabilitation of oropharyngeal dysphagia in children with cerebral palsy. Of the 22 reports, 12 (54.5%) were from Canada, 3 (13.6%) were from Japan, 2 (9%) were from Brazil, 2 (9%) were from Germany, 1 (4.5%) was from the USA, 1 (4.5%) was from the United Kingdom, and 1 (4.5%) was from Poland. Of these reports, 63.6% used oral sensorimotor therapy as a therapeutic method, 36.3% reported continuing education as a therapeutic approach, and only 18.1% and 9% used the Bobath concept and Castillo Morales concept, respectively. CONCLUSION: Even with a constantly increasing cerebral palsy population, few studies include (re)habilitation in the treatment of oropharyngeal dysphagia in these children.
